Introduction
A root cause is defined as a factor that caused a nonconformance and should be permanently eliminated through process improvement. Root cause analysis (RCA) is defined as a collective term that describes a wide range of approaches, tools, and techniques used to uncover causes of problems.
RCA methods and tools are not limited to manufacturing process problems only. Many industries are applying RCA methodology in various situations and are using this structured approach to problem solving. Some examples of where RCA is being used include, but are not limited to:
- Office Processes and Procedures;
- Quality Control Problems;
- Healthcare Incident Analysis;
- Safety-based Situations or Accident Analysis;
- Failure Analysis in Engineering and Maintenance;
- Change Management or Continuous Improvement Activities;
- Computer Systems or Software Analysis.
This session will enable participants to understand root cause analysis as a procedure for ascertaining and analyzing the causes of problems in an effort to determine what can be done to solve or prevent them. Consisting of theory and breakout sessions, this course is designed to provide attendees with an in-depth understanding of how to analyze a system to identify the root causes of problems.
Programme Objectives:
After the programme, participants will be able to:
- Enhance problem solving effectiveness by providing a model for more deeply analyzing problem situations.
- Clarify the difference between analytical and creative thinking, and when each is most useful.
- Promote the ability to provide problem-solving support in situations where one is not an expert in the process or technology involved.
- Expand the range of tools available for analysis of problem situations.
- Identify the need for and successfully execute Cause Analysis (three types), Creative Problem Solving, Decision Making and the Anticipating Problems and Planning processes.
- Demonstrate critically important Group Effectiveness and Teamwork skills.
- Increase creativity in problem solving by pushing through mental roadblocks and broadening existing thinking to create innovative ideas and solution.
- Reduce Decision Making Risks – Financial, Inaction, Intangible and Achievement Risks.
- Resolve fully, or partially one or more work-related concerns.
- Create an Action Plan to further develop Problem Solving & Decision Making skills in the context of work-related issues.
